Questionnaires are a great way of interacting and getting information that can be used for several productive purposes. The Growth99+ application allows its business account users to create custom questionnaires for their patients. Information regarding the patient's general health, symptoms or information that the doctor needs before performing any treatment for the patient, post-service feedback may be obtained via these questionnaires.
When a patient books an appointment for any service, the business account user may assign questionnaires relevant to that service to the patient. Let us learn how to assign a questionnaire to a patient.
STEP 1
Using your login credentials access your Growth99+ business account. Click on the sub-menu named "Patients" below the menu of "Accounts & Settings". Click on the option named "Patients" in the drop-down menu. Refer to the below image for more details.
STEP 2
You will be redirected to a new page that shows a list of the patients who have booked an appointment with your clinic. A "blue icon" next to the patients' names allows you to edit or add information for that particular patient. Click on the pencil icon, next to the patient's name to whom you want to assign a questionnaire. Refer to the below image for more details.
STEP 3
You will be directed to a page that has several tabs for consents
(To learn how to assign consents to patients CLICK HERE) and payments among other things. Click on the tab titled "Questionnaires" to find all the questionnaires that have been previously assigned to the patient. If no questionnaires are assigned to the patient, the page will not show any data. Click on the blue "Assign New Questionnaire" button as the below image highlights.
STEP 4
Once you click on the button for assigning a new questionnaire, you will be redirected to a page that resembles the below image. Choose the questionnaire you want to assign to the patient and proceed to click the "Send to Patient" button. In the below image we have used the example of assigning the post-care questionnaire to the patient.
Once you click the "Send To Patient" button, the patient will receive the questionnaire on their registered email ID.
