Informed consent is an important part of several medical and surgical procedures. An informed consent contains details of the medical/surgical procedure, any potential risks, and important instructions patients must follow before and after the procedure. Every consent form has information customized according to the service that it is intended for.
The Growth99+ application has a menu in its admin panel which allows pre-written consent forms to be sent to patients when they book an appointment. These consent forms have pre-defined variables which get auto-filled with details like the patient's name and the date of the appointment before the patient receives them. This saves a lot of time that the doctor/clinic would have to spend in manually adding the details for each patient before sending out the consent form via email.
Let us see how consents for specific services are assigned to patients.
STEP 1
Use your login credentials to access your Growth99+ business account. Click on the sub-menu titled "Patients" below the menu of "Appointments". Click on the option titled "Patients" in the drop-down menu of "Patients". Refer to the below image for more details.
STEP 2
You will be redirected to a new page that will display a list of all the patients who have booked an appointment with your clinic. A "blue pencil icon" next to every patient's name allows to edit or add information for that particular patient. Click on the pencil icon, to whom you want to assign consent. Refer to the below image for more details.
STEP 3
When you click on the pencil icon, you will be directed to a page that has several tabs for assigning consents and questionnaires (To know how to assign a questionnaire to a patient
CLICK HERE), payment details and other details. Upon clicking on the tab titled "Consents" you will be directed to a page shown in the below image. Here you will find a list of consents previously assigned to the patient for services they have availed with the clinic previously. On the right top corner of the page, you will find a blue
"Assign New Consent" button.
STEP 3
When you click on the "Assign New Consent" button, you will be directed to a page as shown in the below image. Select the service/services for which you want to assign consent to the patient. In the below image we have used examples of assigning Ulherapy Consent Form, Plasma Pen Consent Form, Microdermabrasion Consent Form to the patient.
Do not forget to click the "Send to Patient" button to assign the consent to the patient.
The patient will receive the assigned consent on their registered email ID.
